Updated Case Submission Form
We ask you to download the updated Case Submission Form for a better service. From 2018, the following words will be substituted in the reports and Case Submission Form:
- Patient instead of Animal
- Person responsible instead of Owner
- Name/ID instead of ID (ID = identification)
Please, we kindly ask you to delete the old Case Submission Form from your computers.
